Try:

Practical English Usage, by Michael Swan
It doesn't have exercises, though.

Essential Grammar in Use Edition With Answers: A Self-Study Reference and Practice Book for Elementary Students of English, by Raymond Murphy

Murphy has other books in the same series for intermediate and advanced students. Look for them at any online bookstore.

You may also consider the books by Betty Azar, such as:

Basic English Grammar, Third Edition (Full Student Book with Audio CD
and Answer Key) (Jul 28, 2005) by Betty Schrampfer Azar and Stacy
A. Hagen

which also appear in a series, and have the companion site with forum:
http://thegrammarexchange.infopop.cc/groupee/forums/a/frm/f/340600179

I'd start with Swan.

Also, consider online:

Darling's Guide to Grammar and Writing
http://grammar.ccc.commnet.edu/grammar

Also, the classical, but obsolete at times:

Fowler: King's English, 1903
http://www.bartleby.com/116/index.html

British Council: Grammar

BBC: Learn it - archive

Discourse and Grammar in English: Erdmann (Advanced)

And our own site has basic grammar tutorials with exercises right here (http://www.englishpage.com/grammar/).
