He wished to die in the country where his son belonged. (No "to" at all).

Where is an adverbial pronoun that heads a relative clause. It is used to replace 'to which' or 'which...to':
He wished to die in the country to which his son belonged.
He wished to die in the country which his son belonged to.
To which=where
So no 'to' is needed.
The same applies for in which:
In which=where
This is the house, in which I used to live.
This is the house, which I used to live in.
This is the house, where I used to live.
